My fails were buying DVDs that came with packaged with equipment that I already owned. For example, a few times I couldn't get a DVD I wanted unless I bought it packaged with a stability ball, or a yoga DVD unless I bought it packaged with a strap. It's not that the equipment was a fail - I do like using a stability ball and a yoga strap is very useful - but how many multiples of them does one person need?!

I don't know that I have a "fail" per se but I don't use my BOSU all that much. However, I just found out that my DH will be using it for exercises assigned to him by his physical therapist to help his healing foot. She wouldn't have given them to him if we hadn't had one at home and he saw a benefit when he did the exercises in his last session. So boom! Potential fail becomes potential success!
