Depending upon the word processing software you have on your computer, there might be a template you can just print out. I have Word 2013 and there are exercise logs that you can just click and print.

One of the commercial options I have tried that I used the most is The Fitbook - It's pricey, but for some reason it works for me.

Alta:
For strength training I use a log that has columns for the following:
Left to right:

Movement
Weight
Sets and Reps
Time
Volume
Density

I time all the exercises including the rest periods. So I start the timer right before my first set and stop it after my last set. I track intensity (weight), Volume (weight multiplied by reps) and density (volume/time)

I try to always do better than the last time I worked out in either intensity, volume or density.

ETA: I have double sided copies of the log I described above and keep them in a flexible 3 ring binder which I take to the gym with me. You could just take the sheet alone if you wanted to. I am just anal about seeing all my stats from previous workouts and comparing right away. Oh and the bottom of the sheet has a section titled "notes" where you can log other activities such as cardio.

For years I've just used spiral-bound student planners that have a week on 2 pages. There's usually plenty of room to record which video I've done and any other relevant info, like weights used, reps done, how I felt, etc., and some have covers you can slide a rotation calender or whatever else into.

They're usually pretty inexpensive this time of year, too.

I always log my workouts on a motivational calendar in my exercise room.

At the end of each month, I total up the number of exercises per category that I track: weights, cardio, yoga, dance, walk, core. At the end of the year, I total those. It's quick and easy and very visual, much more satisfying to me than electronic logs.
