Tracie Long comes to mind, her TLT and Longevity Series. Defining Shape has a balance segment after the weights. It's one of my favorites.

I have these listed as balance workouts in my word documents. There are not all balance per se, but I'm sure that there is balance work involved. I also have lots of Jane Adams, Classical Stretch, and Essentrics listed. I can list specific episodes if anyone is interested.
My apologies if some of these involve foot and ankle work. In my word document, I have them all listed together, since I have heard that strong feet and ankles are essential to having good balance.

The Firm - Skills, Drills & Thrills with Tina Smalley. I haven't done it in awhile, but I remember finding it challenging.

It came with a green balance disc, which I'm sure isn't sold with the workout anymore, but I would think any disc would work?

Lee Holen recently put out a DVD - QiGong for Better Balance. I think is it easier than Jane Adams or V-Core. To make it harder I closed my eyes during most of it today. In the future I plan to try using BOSU or Stability disks.

All of the Balletone workouts have primarily a balance focus

Kari Anderson has some good balance workouts: Center Floor, Angles, Lines & Curves (1 & 2) and Reach.

Jessica Smith 10 pounds down Cardio Abs
has two workouts with balance: "Total body abs transformation" has balance incorporated similar to her WS1 set. "Concentrated Abs" has a warm-up with balancing moves.
